The western terminus of the route is at an intersection with the concurrency of NY 13 and NY 38 in the village of Dryden in Tompkins County. Its eastern terminus is at a junction with U.S. Route 11 in the hamlet of Messengerville within the Cortland County town of Virgil. NY 392 passes through the hamlet of Virgil, where it intersects NY 215.
