Its southern terminus is at U.S. 220 Alternate near U.S. 220/Future I-74/Future I-73, exit 149. From there, the route loops northeast as Zoo Parkway towards the zoological park. Near the park entrance, a spur route named Spur NC 159 splits from the main road and travels to the Zoo`s main gate. The normal route continues northwest after the SPUR NC 159 intersection to its northern terminus at US 64/NC 49 in Asheboro.
