It runs though generally rural areas of east central Columbiana County from downtown Lisbon, Ohio at the intersection of U.S. Route 30 and state routes 45, 164, and 517 to the Pennsylvania state line near Negley, Ohio where it is redesignated as Pennsylvania Route 251. The route continues toward Chippewa, Pennsylvania.
