The highway begins on a bridge over the Des Moines River within sight of its mouth at the Mississippi River. It runs above the northern bank of the Mississippi towards downtown Keokuk. There, it turns southeast where it meets the southern end of US 218. The highway leaves Iowa on the Keokuk–Hamilton Bridge over the Mississippi River.
